Application Form, Examination Rules, Exam Provider Disclosure, and Equipment Requirements

• Examinations are for individual medical physicists considered by IMPCB to have met the education and training requirements.
• Please refer to the general requirements shown on the previous pages.
• Applicants must review and agree with the Online method by signing the Disclosure form you will receive after expressing the interest to choose the Online Exam Option.
• Include the Disclosure form with the required documents to complete the application process.
• The documents to be submitted must be named appropriately, clearly legible, and file sizes must be under 0.5 MB each as described in the Application form and checklist.
• The Name Declaration form is needed if your transcripts and other important documents do not bear the same name as in file #02.
• The application process may take more than one month if the required documents are missing. You may not be admitted to sit the upcoming Exam Session due to the limitation of seating.
